| Eligibility Requirements: |
| All UNO classified, non-classified, full-time or part-time faculty and staff are eligible for the UNO Employee of the Month and Year awards. |
Eligibility Criteria: |
| Excellence in UNO job performance, initiative, attitude, involvement, responsibility, dependability, customer interaction and cooperation. |
Nomination Format: |
| Typed or written correspondence, not to exceed two (2) paragraphs, supporting the nomination based on eligible criteria. |
Nomination Procedure: |
| Nominations should include nominee's full name, department, phone number, supervisor, the same information for the nominator, and supporting information based on above criteria or by completion of the nomination form. |
| UNO faculty and staff may submit nominations. |
| All nominations submitted should be brought to the attention of the supervisor. |
| Nomination deadline is the 1st of each month. |
| Two employees will be recognized each month. |

| Employees of the Month Recipients will receive the following: |
| High Note Member pin, High Note Polo Shirt and a certificate acknowledging your award. |
| Picture and brief story of accomplishments to appear in the UNO Now Notes and on the UNO Web Site. |
| Picture displayed in the High Note Employee of the Month display case located in the University Center. |
| Reserved parking for one month. |
| Voodoo Bar-B-Que gift certificate in the amount of $25.00. |
| Public recognition at an athletic event in January and at the Employee Awards Ceremony in May. |
| Eligible for Employee of the Year Award. |
From the pool of monthly recipients, two employees will be recognized as the High Note Employee of the Year at the annual Employee Awards Ceremony in May.
| Employees of the Year will receive the following: |
| Cash award of $500, plaque acknowledging the award and a High Note Jacket (reversible and water repellent). |
| Gift Certificate for one night stay at the Renaissance Pere Marquette Hotel. |
| Dinner for two at a downtown restaurant. |
| Tickets to a UNO Kiefer Lakefront Arena event subject to availability and scheduling. |
| $100 in “arena bucks” redeemable at UNO Kiefer Lakefront Arena concession stands. |
| Tickets to an Athletics event subject to availability and scheduling. |
| $200 voucher from Metro College. |
| Family photo provided by the Department of Creative Services. |
Any department that meets the High Note criteria will receive a sign somewhere in the department that is highly visible. The sign will say:
This is a High Note Office…Customer Satisfaction is our #1 Job |
| High Note Department Winner Criteria: |
| Department must have 100% of eligible employees trained in Service with Jazz Program. |
| Department must have, at least, two monthly winners. |
| Department must have an active Customer Satisfaction Survey as part of its Institutional Plan. |
| Department must reach stated level of expected Customer Satisfaction in its I.E. Plan. |
